1. What personal information do we collect and how?
We collect such types of information as we reasonably require for the provision of our services,
in accordance with this policy or as may be required by law. We will require and collect your
personal information, such as name, address, telephone number, e-mail address and social
insurance number. We will also collect financial information provided by you or the Canada
Revenue Agency (or other provincial government agencies), medical information provided by
you or health professionals you have authorized us to deal with and credit card information. We
collect this information by means of personal interviews and the physical receipt of such
documents as confidential tax and medical records.
2. Why do we collect specific personal information?
We collect your personal information in support of your application for disability-related tax
credit and/or benefits and subsequent access to various federal and provincial disability
programs. All information is used to provide our services, bill our clients and collect our fees.
We will not disclose your information except as specifically set out in this policy and for the
purposes above.
3. How do we securely store your information?
We protect your information from loss, unauthorized access and disclosure by employing
appropriate security measures with respect to our premises and our technology systems. Access
to the information is restricted to those employee and consultants who require it in connection
with the uses set out in this policy.
4. Who has access to your personal information?
Only a limited number of employees have authorized access to your confidential information,
usually: i) The employee who conducts your initial interview and receives your confidential
information, tax and medical records on behalf of the company; ii) Our medical liaison who
reviews your file and liaises with your doctor and the Canada Revenue Agency; iii) Our Accounting / Tax professional who processes your retroactive income tax amendments. If any
further access is necessary, you will be advised.
5. Who do we share your personal information with?
We only share your personal information with the following: i) the primary contact or the
disabled individual with respect to the client file that relates to you, ii) health practitioners in
connection with the provision of our services, to the Canada Revenue Agency or provincial
agencies in connection with the provision of our services, iii) service providers that we may
retain from time to time, such as, but not limited to, data back up or credit card processing
providers, disclosing only information that is necessary for the performance of such services. If
we believe that further disclosure is necessary, we will seek your prior permission. However, we
do obey lawful orders and/or warrants to produce personal information if properly prepared and
served upon us.
6. How do we use the specific personal information collected?
We use your information strictly to support our efforts to assist you in the application process for
disability-related tax credit and/or benefits and appropriate retroactive income tax relief and
subsequent access to various federal and provincial disability programs. Included within that
mandate is liaising with your medical practitioner to ensure that the medical evidence recorded
on your completed application by your attending physician satisfies the legislative requirements
as to form and content.
7. Do I have a right to review my own information and/or make corrections?
